NURS 4462 | Community Health Nursing | Clinical Practicum
- This practicum focuses on nursing care of populations within communities and encompasses principles of primary care nursing; public health nursing; community health nursing; social determinants of health; epidemiology; Healthy People 2030; diversity, equity, inclusion, and access; evidence-based practice; disaster preparedness; program and project management; leadership; health promotions; disease prevention; and harm reduction.
- Clinical practicum experiences include direct patient care and the collaborative accomplishment of a community health project.
- Students’ responsibilities include the following:
- Accomplish required clinical background check.
- Maintain updated immunizations, TB test, and CPR.
- Provide RN preceptor with contact information (e-mail or phone #).
- Comply with UIW and clinical site identification (ID) and dress code policies.
- Provide own transportation (i.e., not permitted to travel with faculty or RN preceptor).
- Use cell phones for professional purposes only.
- Accomplish clinical evaluations as instructed.
- Collaborate clinical make-up hours with both the RN preceptor and clinical faculty.
- Demonstrate professional and ethical conduct consistent with Standards of Community Health Nursing (CHN) Practice, the American Nurses Association (ANA) Code for Nurses and the Nurse Practice Act.
- Apply knowledge of biophysical, psychosocial, cultural, and environmental factors in assessing the needs of populations.
- Analyze the interrelatedness of demographic and contextual factors affecting community health needs of the community.
- Develop client-centered and community care plans emphasizing the three levels of prevention (i.e., primary, secondary, tertiary) to address specific community health problems.
- Implement evidence-based community health care for individuals, families, and groups and populations focusing on primary prevention.
- Evaluate the effectiveness of community-based nursing care, comparing predicted and actual outcomes.
- Apply research findings in planning and implementing care for community clients.
- Guide community clients to become advocates for their own health.
- Analyze the effects of the environment and clients’ personal values and behaviors on the ability to care for self and family.
- Explore the multiple roles and functions of the community health nurse in improving the health of clients.
- Work collaboratively with interdisciplinary healthcare team and key professionals in the community to improve the delivery of health care services and the health of the community.
- Comply with scope of practice limits and seek supervision appropriately.
Community Health Nursing Frequently Asked Questions
The student will be reassigned by their agency’s coordinator or clinical faculty to an appropriate alternate RN preceptor.
The RN preceptor should notify clinical faculty anytime a student is not meeting professional expectations.
